Why Did Antidote Use Before Referral Track With More Severe Poisoning?
A treatment can appear more often in severe cases because clinicians select it when patients are already sicker. That possibility matters when a poisoning study compares people treated before referral with those first treated at the receiving hospital.
What counted as treatment before admission
The Isfahan study examined 300 patients with acute poisoning at a referral center. Its methods included patients who had received treatment at another center in the group treated before admission. The authors found that antidote administration before referral-center admission was associated with greater poisoning severity.
What to reconstruct from the chart
The study was observational. The association could reflect the circumstances that prompted antidote treatment or referral, including symptoms that were already present. To evaluate a particular case, establish the patient’s condition before the antidote, the stated indication, and the timing of later findings. The study cannot show from the association alone whether the antidote made the poisoning worse.
