Neonatal Injury With Multiple Prenatal Exposures: Benzodiazepine Attribution Was Unresolved

A neonatal case report described severe abnormalities after several reported maternal exposures, without isolating a benzodiazepine effect.

A neonatal case report described severe abnormalities after several reported maternal exposures, without isolating a benzodiazepine effect.

The reported presentation

A preterm newborn had musculoskeletal abnormalities, sensory concerns, respiratory distress, and withdrawal-like symptoms. The mother reported buprenorphine, benzodiazepines, gabapentin, and heroin use. The authors considered polysubstance exposure and limited prenatal care relevant to the complex presentation.

A severe case is not a drug-specific risk estimate

The report cannot identify the independent contribution of any one exposure or separate every prenatal, hypoxic, obstetric, and postnatal influence. Reported use also does not fully specify dose and timing. The authors’ attribution remains an interpretation of one case, not a controlled comparison. For forensic review, distinguish observed abnormalities, documented exposure, and hypothesized mechanism. The paper cannot establish that benzodiazepines alone caused the injuries or quantify risk for another pregnancy with different treatment and clinical circumstances.
