Benzodiazepines as Possible Contributors in a Clozapine Hypothermia Case
A hypothermia report considered several drugs and diseases rather than establishing a single definitive cause.
A hypothermia report considered several drugs and diseases rather than establishing a single definitive cause.
The observed event
A 70-year-old woman developed marked hypothermia during initial clozapine titration, with cardiovascular and respiratory abnormalities. Several medicines were discontinued and she was rewarmed. The authors considered clozapine, benzodiazepines, other psychotropics, antihypertensives, and comorbidity as potential contributors.
A causality score retained uncertainty
The reported Naranjo classification for clozapine was possible, not definite. Withdrawal of several medicines at once prevents a clean drug-specific dechallenge interpretation. The presence of benzodiazepine treatment therefore does not establish its causal share in the temperature change. This case supports reviewing thermoregulation, co-medications, infection and endocrine evaluation, and timing together. It cannot estimate hypothermia incidence or prove that one sedative independently caused the event. The remaining uncertainty is part of the finding rather than a detail to omit.
