How Plasma Exchange Can Complicate a Clobazam Concentration Timeline
A single patient with new-onset refractory status epilepticus received multiple antiseizure medications and underwent therapeutic plasma exchange. The authors recorded serum concentrations before, during, and after two exchange sessions. That sampling sequence is useful when deciding whether two concentrations describe comparable clinical moments.
What the case recorded
In the report, clobazam fell to approximately 85% of the starting concentration during exchange and to approximately 75% after exchange at the lowest observed points described. Levetiracetam and topiramate also fell. A small series of samples during one patient’s treatment cannot separate every contribution from procedure timing, dosing, or redistribution.
How to reconstruct the timeline
Align each laboratory draw with the dose and the exchange start and stop times. Identify whether the measurement was for parent clobazam, a metabolite, or both. A concentration decrease during this case makes timing relevant to the interpretation. It supplies no general correction factor and cannot establish that a later seizure was caused by the decline.
