Mixed-Drug Death Report Attributed Primary Toxicity to Paroxetine With Alprazolam Co-Exposure
A postmortem case described paroxetine, alprazolam, and antipsychotic findings in a young adult. The authors’ mixed-intoxication conclusion rested on the combined investigation rather than benzodiazepine detection alone.

The evidence assembled
Florou and colleagues described a 22-year-old man found dead with medication packaging at the scene. The investigation included history, autopsy, scene information, and toxicology of external-iliac blood, urine, and gastric contents.
Blood paroxetine was reported at 3.3 mg/L. Alprazolam and its metabolite, quetiapine, and a urinary clozapine metabolite were also reported. The authors regarded paroxetine as the primary toxicant and the other medication findings as contributing context.
Detection and contribution are different propositions
The paper described alprazolam within a therapeutic reference range and quetiapine below its therapeutic range. Those labels did not make the authors disregard combined effects, but neither do they prove synergy of a particular magnitude. The report did not experimentally isolate the contribution of each drug.
A urinary metabolite supported prior exposure, without supplying a precise dose or timing of ingestion. The distinction between the sampled compartments is essential.
Use in a forensic opinion
Present the causal attribution as the authors’ case-specific assessment. One mixed-drug death cannot define a universally lethal concentration or establish that every detected medicine materially contributed. Explain how scene, medical, analytical, and postmortem evidence fit together before drawing an individual conclusion.
