Recovery-Quality Meta-Analysis Found a Physical-Independence Signal Without a Clear Overall QoR-15 Benefit
A recovery meta-analysis distinguishes an improved questionnaire component from an uncertain overall recovery-score difference.
A meta-analysis comparing remimazolam and propofol did not demonstrate a statistically significant improvement in the overall Quality of Recovery-15 score on the first postoperative day. One component, physical independence, favored remimazolam. Those two findings should be reported together.
The pooled findings
The review included 13 randomized trials with 1,305 patients. For total day-one QoR-15, the reported mean difference was 1.24 points, with a 95% confidence interval from −1.67 to 4.15 and substantial heterogeneity. The physical-independence component favored remimazolam by 0.79 points. Hypotension, bradycardia, and injection pain were less frequent in the pooled remimazolam comparisons.
A component result is not the total score
A favorable dimension of a recovery questionnaire can be meaningful without proving a general improvement across all dimensions. The nonsignificant total-score result likewise does not prove the treatments are identical. Its confidence interval describes uncertainty around the pooled difference. Clinical importance also depends on the scale and context, not simply whether a P value crosses a threshold.
What limits an expert’s conclusion
The included populations were Asian, age ranges varied, and the authors could not determine elderly versus younger differences adequately. Trials also differed in protocols and surgery. The review therefore does not establish superior global recovery for every patient, and a day-one patient-reported questionnaire is not a direct assessment of immediate post-sedation driving or legal decision-making capacity.
