MERRF Clonazepam–Levetiracetam Series Was a Sequential Comparison in Selected Nonresponders
An improvement after adding treatment is not the same design as a randomized parallel comparison.
An improvement after adding treatment is not the same design as a randomized parallel comparison.
What the publication shows
The report followed seventeen patients with genetically characterized MERRF. Twelve with poor responses to initial monotherapy later received levetiracetam plus clonazepam, with favorable reported antimyoclonic outcomes. Some also had improved cognition and coordination.
Limits of the evidence
The groups overlapped in time and membership, and the combination group was selected after earlier treatment response. Follow-up periods differed substantially. Small numbers, heterogeneous prior treatment and an internally inconsistent denominator in the report limit precision. No specific dose should be inferred from the summary alone.
Application to expert review
For appraisal, reconstruct which patients contributed to each stage and how response was defined. Do not treat twelve favorable outcomes as an unbiased population success rate or proof that clonazepam alone produced the improvement. Preserve the rare genetic disease context.
