Dementia Agitation Cohort Shows the Problem of Prescribing to Sicker Patients
A hospital association between benzodiazepine exposure and greater agitation cannot establish which came first.
A hospital association between benzodiazepine exposure and greater agitation cannot establish which came first.
What the supplied paper reports
The retrospective study included 1,709 patients with dementia, of whom 680 had delirium. Agitation was measured during the first 72 hours with the Pittsburgh Agitation Scale. Psychotropic prescribing was more frequent with delirium, and lorazepam and alprazolam were among the drugs associated with higher agitation scores in regression analyses.
What the findings cannot establish
More agitated or medically unstable patients may be more likely to receive treatment. This confounding by indication, together with reverse causation, prevents a simple claim that the drugs produced the agitation. Discharge documentation may also miss delirium, especially its quieter forms.
Implications for forensic review
An expert should reconstruct baseline behavior and individual administration times before interpreting a group-level association. Compare agitation before and after each intervention, oxygenation, falls, functional status and other medicines. The study supports better monitoring and a careful differential explanation, while leaving the causal effect of a particular benzodiazepine unresolved.
